Job Title: Part-Time Physician Assistant (PA) or Nurse Practitioner (NP) – Diabetes Care & RPM 

Location: Remote / Telehealth 

Position Type: Part-Time (2–4 hours/week to start) 

Clinic: OC Diabetes and Endocrinology 

About Us: 

Our clinic is located in Fountain Valley, CA, focused on providing expert care in diabetes and endocrine disorders. We are expanding our services to include Remote Patient Monitoring (RPM) for patients using Continuous Glucose Monitors (CGM), and we're looking for a dedicated provider to join our team on a part-time basis. 

Position Summary: 

We are seeking a Physician Assistant (PA) or Nurse Practitioner (NP) with experience in diabetes management and interpreting CGM data. This position will involve reviewing patient CGM data, making treatment adjustments, and documenting RPM services for billing compliance. Initial commitment is 2–4 hours per week, with the potential to grow. 

Responsibilities: 

Conduct telehealth check-ins with patients using CGM 

Analyze CGM data and make appropriate treatment recommendations 

Collaborate with supervising endocrinologist 

Document clinical encounters and RPM activities per Medicare guidelines 

Requirements: 

Fluent in Vietnamese 

Current PA or NP license and board certification 

At least 1 year of experience managing patients with diabetes 

Proficiency in CGM interpretation (e.g., Dexcom Clarity, LibreView) 

Preferred: 

Experience with EclinicalWorks EHR 

Comfortable with telehealth platforms and RPM workflows 

Compensation: 

Hourly rate, commensurate with experience

FLSA Status: Exempt

Wage range that the company expects to pay: $5,576.92 - $6,148.56 bi-weekly

SUMMARY: Under the direction of the Medical Director, but with independent responsibility for professional results the NP/PA provides primary healthcare services for Community Health Centers of the Central Coast, Inc. (CHC), and to perform related work as required. We seek a skilled and compassionate clinician. The ideal candidate will have a strong background in diagnosing and managing endocrine disorders with a strong focus on diabetes, thyroid disease, and obesity.

It is the primary purpose of CHC to provide the highest quality of total care possible to the patient population it serves. Such a level of quality depends ultimately on the staff’s desire and ability to work together, individually, and as a team. The employee is expected to be professional, punctual, maintain regular attendance, cooperative, organized, and enthusiastic at all times.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. EDUCATION and/or EXPERIENCE: Completion of an accredited Physician Assistant Program. Completion and passing of the Physician Assistant National Certifying Examination.

CERTIFICATES, LICENSES, REGISTRATIONS: Possession of current unrestricted and valid License – NP/PA and Surgeon or equivalent from Medical Board of California. Board Certified/Eligible Family Practice, Internal Medicine, OB/GYN or Pediatrics, required. Possession of current unrestricted and valid DEA license, Advanced Cardio Life Support (ACLS) for FP/IM, Pediatric Advanced Life Support (PALS) card for PEDS; and Basic Life Support (BLS-C) card for all specialties. Annual health examination; annual TB skin test clearance or biannual chest x-ray, and up-to-date immunizations for health care workers at time of hire and annually thereafter.

The Division of Endocrinology is a vibrant and growing team of physicians, advanced practitioners, nurses, diabetes nurse educators, nutritionists and support staff.  While we see a variety of patients in our hospital and clinic-based practices, our focus is on rare and complex endocrine disease.  The Endocrinology APP Supervisor will oversee seven APPs across two campus locations.  The ideal leader will have demonstrated abilities in developing staff and building a cohesive and collaborative culture during times of great change and growth.
